There was oil on the isles floors. This condition exposed employees to slip and fall hazards.

On or about August 25, 2016, employer did not ensure to remove and replace defective personal fall protective equipment. One harness was defective and missing the front strap latching device.  The other was damaged and deteriorated. This condition exposed employees to fall hazards.

These conditions exposed employees to struck-by hazards.   a) Forklift #1- no brakes, horn not operative, leaking oil.  b) Forklift #6- no brakes, horn not operative, leaking oil.  c) Forklift #7- no brakes, leaking oil.
