29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  (a) On or about August 22, 2016, at a jobsite located at 1100 East 63rd Street, Kansas City, Missouri, the employer failed to require that an open-sided floor/platform four feet or more above ground level be guarded with standard railings (or equivalent). Employees were exposed to a serious fall hazard of approximately 20 feet above ground level when conducting repair work on a rooftop cooling unit. An employee fled from a flash fire on a path that took him within 5 feet of the roof edge.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.252(a)(2)(ii): Suitable extinguishing equipment was not maintained in a state of readiness for instant use:  (a) On or about August 22, 2016, at a jobsite located at 1100 East 63rd Street, Kansas City, Missouri, the employer failed to maintain suitable extinguishing equipment in a state of readiness for instant use. Employees were exposed to fire hazards in that brazing operations occurred when combustible R-22 refrigerant could not be removed from the immediate area. An employee suffered second and third degree burns to the face and arms when refrigerant was ignited. A fire extinguisher was not immediately accessible at the time of the fire.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.252(b)(3): Employees exposed to the hazards created by welding, cutting, or brazing operations were not protected by personal protective equipment in accordance with the requirements of 29 CFR 1910.132:  (a) On or about August 22, 2016, at a jobsite located at 1100 East 63rd Street, Kansas City, Missouri, the employer failed to protect employees exposed to the hazards created by welding, cutting, or brazing operations with personal protective equipment in accordance with the requirements of 29 CFR 1910.132. Employees were exposed to fire hazards in that brazing operations occurred when combustible R-22 refrigerant could not be removed from the immediate area. An employee suffered second and third degree burns to the face and arms when refrigerant was ignited. The employee performing brazing operations wore safety glasses and leather gloves.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
