On or about 8/22/2016, employees were operating defective powered industrial trucks.  The following forklifts were on site and not removed from service:  a)            Komatsu Forklift #8, Model FG25St-14; the LPG tank indexing/locator pin was missing  and the tank was installed incorrectly.  b)            Komatsu Forklift # 3, Model FG35BCS-7; the LPG tank indexing/locator pin was missing, the LPG tank was installed incorrectly, the operators seat was damaged and missing padding and a pedal was missing the pad or cover.  c)            Komatsu Forklift # 10, Model FG25ST-14; the LPG tank indexing/locator pin was missing.  d)            Komatsu Forklift # 14, Model FG25ST-16; the LPG tank indexing/locator pin was missing.  e)            Toyota Forklift #2, Model 8FGCU25; the LPG tank locking pin/bracket was damaged.    f)             TCM Forklift #24, Model FCG368L; one of the brake lights was inoperable and the LPG tank indexing/locator pin was missing.    g)            TCM Forklift #23, Model FCG30HL; the LPG tank indexing/locator pin was missing and a pedal was missing the pad or cover.
