29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: a) On or about 8/23/2016, a positioner was being used in the Sub Arc #3 area of Carbon Bay #4 which had a gear fail, allowing a pipe to swing free striking an employee. b) On or about 8/26/2016, employees were using 5 ton capacity rollers beyond the manufacturers rated load capacity. Employees were using two rollers to fabricate blow down tanks that weighed approximately 16 ton.
