29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides except where there is entrance to a ramp, stairway, or fixed ladder:    (a) Iron Mountain Inc, at 3900 Nome Street, Unit A, Denver, CO: On and preceding 8/23/16, the employer did not ensure that open sided platforms 4 feet or more above the adjacent floor or ground level were guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.  Employees performed duties in the warehouse, including placement of boxes of documents onto wooden pallets at openings to the second and third floor mezzanine railings.  Gates at the mezzanine openings, approximately 67 inches wide, were tied open during the work shift.  The employer did not ensure that the mezzanine openings were guarded at all times by standard railings.  This condition exposed employees to an approximate sixteen foot fall hazard from the third floor mezzanine.

29 CFR 1910.132(f)(1):  The employer did not provide training to each employee who is required by this section to use PPE:    (a) Iron Mountain Inc, at 3900 Nome Street, Unit A, Denver, CO: On and preceding 8/24/16, the employer did not provide training to each employee who is required by this section to use PPE.  Employees performed duties in the warehouse, including operation of lead acid battery powered industrial trucks, operation of pallet jacks and four wheel dollies, and transport of boxes of documents.  Employees were exposed to roll over or impact hazards to the feet when performing these operations.  The employer did not provide personal protective equipment training to all employees.  This condition exposed an employee to roll over or impact hazards.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:    (a) Iron Mountain Inc, at 3900 Nome Street, Unit A, Denver, CO: On and preceding 8/24/16, the employer did not ensure that the educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was provided to all employees upon initial employment, and at least annually thereafter.  Employees performed duties in the warehouse, including operation of lead acid battery powered industrial trucks, operation of pallet jacks and four wheel dollies, and transport of boxes of documents.  The employer did not review the emergency action plan with all employees.  This condition exposed an employee to a fire hazard.
