29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, holes, or loose boards. First floor - On or about August 25, 2016, the work place and passageway were not kept free of floor holes, protruding steel plates, and loose floor boards. Employees were thereby exposed to trip and fall hazards.

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations: Production area - On or about August 25, 2016, in the facility employees worked around an electrical junction box mounted on a pillar that was exposed to water leaking from an air conditioner. Employees were thereby exposed to the hazards associated with energized electrical equipment and water.
