On 08/25/2016, at a worksite located at 218 E. Lawrence St., Appleton, WI 54911, Omni Glass & Paint, Inc. exposed two Glazier employees to falls of approximately 12.5 feet from the south roof and 14.5 feet from the west roof. Employees were installing pressure plates to the glass work and were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.
