29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.  a) Jobsite: Scaffold 3tf working platform - The employer did not ensure that all working levels of the supported tubular welded frame scaffold were fully planked or decked between the front uprights and the guardrail supports. Employees were performing stucco patch work and painting activities, approximately 19 feet above ground level.    Noted on or about 8/25/16.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.  a) Jobsite: The employer did not train their employees to recognize the hazards that are present when working from a  supported tubular welded frame scaffold, and to understand the procedures to control or minimize those hazards.
