29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) At 9110 King Palm Drive, Suite 100, Tampa, FL 33619 as observed on or about 09/09/2016: 1) For employees assigned to crimping tasks cutting with a Makita saw aluminum frames and then intalling them on cases exposed to a continuous noise level at 136.2 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 92.2 dBA. 2) For employees operating a mitering saws for lip cutting exposed to a continuous noise level at 66.7 percent of the allowable 8 hour TWA (85 dBA). The equivalent dBA level for these dose percentage is 87.1dBA.
