(a)  On August 26, 2016, at the back side of the project site, an 8-foot Werner aluminum stepladder with a damaged spreader was not immediately marked in a manner that readily identifies the ladder as defective, or be tagged with "Do Not Use" or similar language, and was not withdrawn from service until repaired.
