29 CFR 1926.451(g)(1)(ii): Each employee on a single-point or two-point adjustable suspension scaffold was not protected by both a personal fall arrest system and a guardrail system:    (a) Jobsite:  On or about August 25, 2016, an employee was working from a two-point suspension scaffold and he was not tied off.  The fall hazard to the employee was approximately 50 feet to the concrete sidewalk.

29 CFR 1926.503(c)(3): 29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee:   (a)  Jobsite:  On or about August 25, 2016, an employee who was not wearing any personal fall protection while working from a two point suspension scaffold needs to retrained so that he will always use personal fall protection when working from a two point suspension scaffold.

29 CFR 1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion:     (a) Jobsite:  On or about August 25, 2016, all four lifelines were not protected from abrasion or sharp edges as they passed over the metal covered parapet wall.

29 CFR 1926.451(g)(3)(iv): (iv) Vertical lifelines, independent support lines, and suspension ropes shall not be attached to each other, nor shall they be attached to or use the same point of anchorage, nor shall they be attached to the same point on the scaffold or personal fall arrest system:    (a) Jobsite:  On or about August 25, 2016, two employees were working from a two point suspension scaffold and one vertical lifeline was attached to a second vertical lifeline.   The fall hazard was about 35 feet to the concrete sidewalk.
