The employees were not wearing protective hats to protect themselves from falling objects or tools while working below scaffold operations.

The employees were not provided with safety glasses when cutting concrete blocks and rebar.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access:      a) On or about June 14, 2016 and at times prior, employees were exposed to fall hazards of 21 feet while accessing and egressing the scaffold. The employees used the frame structures and the cross-braces to access and egress the scaffold. 29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:      a) On or about August 25, 2016 and at times prior, employees were exposed from falls of approximately up to 21 feet high while working on the scaffold. The scaffold was found with planks that have cracks and some planks were missing. Guard rails and mid rails were missing on the scaffold. The scaffolds and scaffold components were not inspected by a competent person prior to the employees' use. The scaffold had multiple defects that affects its structural integrity.    Theisen Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(f)(3), which was contained in OSHA inspection number 1062889, citation number 1, item number 4(c) and was affirmed as a final order on April 30, 2015, with respect to a workplace located at 5915 Dublin Blvd., Colorado Springs, CO 80923.
