Employees were exposed to the hazard of being struck by blade and/or caught in ingoing nip point.       b)  August 25, 2016, Shop Area, employees were exposed to crushing and amputation hazards where the employer did not provide guarding to prevent the operator from placing hands in the danger zone when using the following machines:    Accurpress Press Brake Model No. 710010, Serial No. 6041, 100 ton  Accurpress Press Brake Model No. 717512, Serial No. 6388, 175 ton
