29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) August 25, 2016, Shop Area, unused portion of blade on the Jet band saw was not guarded. Employees were exposed to the hazard of being struck by blade and/or caught in ingoing nip point. b) August 25, 2016, Shop Area, employees were exposed to crushing and amputation hazards where the employer did not provide guarding to prevent the operator from placing hands in the danger zone when using the following machines: Accurpress Press Brake Model No. 710010, Serial No. 6041, 100 ton Accurpress Press Brake Model No. 717512, Serial No. 6388, 175 ton
