29 CFR 1910.119(e)(7): The employers did not retain process hazards analyses and updates or revalidations for each process covered by this section, as well as the documented resolution of recommendations described in paragraph (e)(5) of this section for the life of the process. On or about August 24, 2016, Lineage Logistics, Inc., did not maintain the initial process hazards analyses (PHA) and subsequent revalidations, conducted prior to 2009, for the life of the process. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment: On or about August 24, 2016, the employer did not develop and implement written procedures to maintain the on-going integrity of process equipment such as but not limited to compressors. The company does not have written procedures for tasks including isolating the compressors, changing the oil filter and coalescer filters, cleaning out the strainers and changing the oil on the compressors. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
