29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a. Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 25, 2016, the employer did not ensure that the Komatsu forklift, used to move containers of material and bales of metal, was marked to include the front-end rotator attachment showing the capacity and approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  A potential for the forklift to be tipped over as a result of being overloaded exposed employees to crushing hazards.    b. Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 25, 2016, the employer did not ensure that the Hyster forklift, used to move containers of material and bales of metal, was marked to include the front-end rotator attachment showing the capacity and approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 25, 2016, the Hyster forklift did not have a data plate so that operators were provided with information pertaining to the fuel type, forklift weight, and load center capacity to maximum height.

Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 18, 2016, the employer did not ensure that bales of metal handled by forklifts were loaded and moved in a safe and secure manner to prevent the load from becoming removed from the forks.  Metal bales were not secured, exposing employees in the area to struck-by and crushing injuries when the load fell from the forklift.

Employees were exposed to struck-by injuries in that the horn was not operative.      b. Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 25, the employer did not ensure that the Hyster forklift was removed from service during a time in which it was not in safe operating condition.  Employees were exposed to struck-by injuries in that the horn was not operative.

Such examination shall be made at least daily:  a. Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 25, 2016, forklifts were not inspected daily for conditions that adversely affect the truck's safety.  Employees operate forklifts daily to move materials throughout the facility.
