29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a)  At the multi-family residential construction site, where employees were exposed to struck-by hazards while working in areas where overhead hazards existed without the use of head protection.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:   a)  At the multi-family residential construction site where employees were exposed to struck-by hazards while using Makita circular saws to cut wood material without the use of eye protection.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer did not develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502:   a).  At the multi-family residential construction site, employees were exposed to fall hazards of approximately 20-feet above the ground level below while performing punch-out and framing work without the use of a guardrail system or personal fall arrest system.
