29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). Worksite located at 142 Shawnee Mall Drive. Shawnee, OK 74804. On or about August 26, 2016, no protective devices were in place to protect employees working in a trench that was more than 5 feet deep. Employee were exposed to the hazard of being crushed by falling soil. R and M Utilities Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1112781, citation number 01, item number 02 and was affirmed as a final order on March 28, 2016, with respect to a workplace located at NW 150th and Portland, Oklahoma City, OK 73134.
