29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. Employees were exposed to fall hazards in the following areas: On August 25, 2016 on Unit 91 (HERM) employees were cleaning a drip pan while on a beam exposing them to a fall approximately 5 feet 10 inches from the top of the beam to the floor below."

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On the Collection Level, guardrails were removed when the line was down, potentially exposing employees to hazards. Employees also opened doors/panels to clean and remove product that had accumulated during operation. Equipment was stopped but not locked out while using designated tools to perform routine cleaning. Additional machine guarding methods were not in place to prevent employees from being potentially exposed to moving rollers, conveyers, shafts and other moving parts.
