29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: a. On August 19, 2016, at the east end concrete stairs, reinforcing steel bars ranging in height from 6 to 12 inches were not guarded, exposing the workers to impalement hazards.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: a. On August 19, 2016 at the east end of the building, the stairway to the second floor was not protected by stairrails, exposing workers to fall hazards.
