29 CFR 1926.200(h)(1): Accident prevention tags were not used as a temporary means of warning employees of an existing hazard, such as defective tools, equipment, etc. a) Ground Vehicle Fueling Facility - On or about 8/29/16, employees were exposed to electrical hazards when 3-outlet extension cords missing ground prongs were available for use and not tagged out. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Ground Vehicle Fueling Facility - On or about 8/29/16, strain relief was not provided on the power cords of two fans that were being used on-site. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
