Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The employer failed to protect employees from struck by hazards resulting in serious injury. On or about August 12, 2016, at 18201 Wright Street, Omaha, NE 68130, an employee was struck by the chamber door due to the latch not being checked prior to engaging the ram. Among other methods, the following are feasible and acceptable methods to correction: 1. Ensure that employees are understand the manufactures instructions regarding the operation of the baler. 2. Ensure that the employees check the chamber door latch is secure prior to starting the baler per the training video.
