Evidence of a persistent leak from the floor above, included wet paper towels on the floor, water on a countertop, staining on the walls, staining on ceiling tiles and probable visible mold on the wall and between ceiling tiles were visible on or about August 30, 2016.

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. (a) Location: Bottom Floor, Stairwell, The door that led into the ShopRite from Home area; An interior door marked "EXIT" was obstructed by items and equipment; on or about August 30, 2016.

An employee experienced a shoulder strain on July 15, 2016, and had a day away from work, but was not recorded on the OSHA 300 log presented on or about August 30, 2016. An employee was assaulted and sustained bruises and lacerations to her hands and a contusion to her forehead on May 3, 2016, and had 7 days away from work, but was not recorded to the necessary detail, and was not recorded in H or K on the OSHA 300 log presented on or about August 30, 2016.
