Employee(s) working inside of the work zone were exposed to serious struck-by hazards posed by motorists due to a lack of compliance with the Manual Uniform Traffic Control Devices, Millennium Edition, Part VI, as it applied to the use of warning signs, flaggers, or barricades.

29 CFR 1926.200(g)(1):  Construction area(s) were not posted with legible traffic signs at points of hazard:  a/  At the job site, on or about 08/30/16, specifically at the northbound and southbound lanes of Oregon Avenue, no traffic signs were posted at the advanced warning area of the two-day work zone where employee(s) were excavating a pre-existing water pipe in an excavation.  The excavation and the Caterpillar Trackhoe, m/n 304C, were in the southbound lane of Oregon Avenue.  The lack of warning signs provided motorists with little warning of the presence of the two-day old work zone.

29 CFR 1926.202:  Barricades for protection of employee(s) did not conform to Part VI of the Manual on Uniform Traffic Control Devices, 1988 Edition, Revision 3 or Millennium Edition, which were incorporated by reference in 29 CFR 1926.200(g)(2):  a/  At the job site, on or about 08/30/16, specifically the northbound and southbound lanes of Oregon Avenue, no suitable barricades were positioned on either side of the Caterpillar Trackhoe, m/n 340C, where employee(s) were excavating a pre-existing water pipe.  The employee(s) and trackhoe were located in the southbound lane of Oregon Avenue.  Only four, orange cones were posted as the taper line away from the work zone.  The taper ratio for barricades (cones) for a residential, urban street width would require a minimum of eight traffic cones or the equivalent at each end of the work zone.  The lack of cones or the equivalent provided motorists with little warning to steer clear of the two-day old work zone.
