GFCIs and/or assured equipment grounding program were not utilized to protect the extension cords and circular saw.

29 CFR 1926.403(b)(1)(ii): Mechanical strength and durability was not maintained for parts designed to enclose and protect electrical equipment: a.) Mekis Construction Corporation/Construction Site/Bridge Project Near Route 322 & Route 427 --- East Side Abutment Area/Carpentry Operations: On or about July 8, 2016, an extension cord with twist lock style connectors was utilized to power extension cords and a handheld DeWalt Circular saw. A protective cap on the end of one of the connectors was missing. The condition exposed the area where the extension cord's internal conductors entered the twist lock connector.

A section of the extension cord was not outfitted with a strain relief mechanism due to a protective cap that was missing from the end of one of the twist lock connectors.
