29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On 8/29/16, at the Gainesville job site, the employer did not implement Lockout/Tagout procedures as specific as what is required by the OSHA regulation in that the following deficiencies existed in the documented procedures. The procedures did not document: specific machines addressed, list all energies, list all magnitudes of energy, and did not list disconnect locations to eliminate identified energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts, ingoing nip points, rotating parts, flying chips and sparks. On 8/22/16, at the Gainesville facility in the Sheet Metal Department, the employer did not guard press brakes in that Press Brake 4 had light curtains that "muted" when the point of operation was greater than 0.25 inches, allowing operators to insert their hands into the hazard area, without tripping the device.
