29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    East, production, "Jerry can" 68 Ton mechanical press brake: on or about June 1, 2016, operators were hand-feeding metal sheets for stamping components. Installed vertical plastic shield did not prevent operators from reaching into the danger zone during operating cycle due to multiple access openings.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):    Stamping, mechanical power presses - on or about August 29, 2016, employees were assigned to operate some equipment for regular production stamping tasks in the INCH mode due to malfunctioning single-stroke mechanism, and some that were not equipped with adequate safeguarding to prevent access to the point-of-operation. Operators had their hands and fingers exposed to caught-in hazards likely to result in serious injury.     (a) Minster 160 Ton, serial no. 40 - 6 1/2 - 5097: an INCH mode was selected for running regular production stamping tasks because of inoperable or otherwise not functional single-stroke mechanism. Presence sensing device (light curtain) was turned off or otherwise not functional during production. Provided two hand controls were not concurrent.    (b) Niagara 100 Ton, press no. 18, serial no. B - 4 ½ - 48: an INCH mode was selected for running regular production stamping tasks because of inoperable or otherwise not functional single-stroke mechanism. A key for selecting INCH or single-stroke was missing in the workplace. Provided two hand controls were not concurrent. Feasible hand pullback devices were not functional and/or not used.      (c) Rockford Ironworks, press no. 6: two-hand trip cycle actuating device was provided without additional safeguarding (e.g. feasible hand pullbacks and/or restraints) to prevent access during operating cycle.      (d) Unknown press, green: two-hand trip cycle actuating device was provided without additional safeguarding (e.g. feasible hand pullbacks and/or restraints) to prevent access during operating cycle.
