29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Tooling Department - On or about August 30 through September 30, 2016, employees performed servicing and maintenance tasks when performing setups on the following CNC vertical machining centers: a) #2 Excel b) #4 Haas c) #5 Fadal d) #8 Mori Seiki e) #10 Fadal Employees were not required to control hazardous energy sources in the following sequence: 1) turn off and shut down the machines, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) verify that isolation and deenergization of the machines or equipment have been accomplished. Employees were exposed to finger laceration and crush hazards from unexpected start up and/or energy release of hazardous energy. Alternative protection, such as machine guarding, was not adequate. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Knife Bender Dept., Easy Bender X 2 - The knife blade was not guarded where it fed from the decoiler into the bender machine, exposing employees to laceration and amputation hazards in the event of contact with the moving blade. b) Thermoforming Dept., Vacuum Thermoforming Machine #8 - The machine was not adequately guarded on the labeler side of the machine, exposing employees to laceration and amputation hazards at the point of operation of the moving knife press. There is no abatement certification or documentation required for this item.
