29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use. 1) Jobsite - An employee was standing on an elevated platform supported by a RD6-42 Telescopic Handler that was not designed by the manufacturer for such use. The employee was unloading doors while standing on the platform approximately 20 feet from the ground. This condition exposed the employee to fall hazards. Noted on or about 24 AUG 2016.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. 1) Jobsite: An employee was raised approximately 20 feet from the ground by a RS6-42 Telescopic Handler to unload doors without the use of a fall protection system. The employee was standing on a temporary platform that was attached to the forks of the Telehandler. The platform did not meet the fall protection requirements for scaffolds. Noted on or about 24 AUG 2016
