General-duty citation text
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from the hazards associated with broken bones and potential amputations. During routine clearing operation, employees are exposed to these potential hazards from the injection molding machines. The space between the machine and the ejection plate does not have any guarding to prohibit employees from sticking their hands in the area which contains moving parts. This allows employees to place their hands in the area of the machine which closes creating an amputation/crushing hazard. One instance, which occurred on or about August 25, 2016, involved molding machine Model number 1450 in the facility located at 2355 Aspen St, Wahoo, Nebraska.
