On or about August 31, 2016, employer did not ensure that employee(s) working on the roof were provided with and required to use adequate fall protection. Employees were exposed to fall hazards from and elevation exceeding 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about August 31, 2016, employees was observed using a ladder where the side rails did not extend at least 3 feet above the upper landing. This condition exposed employees to fall hazards exceeding 6 feet.
