29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) The employer failed to provide documentation to the Agency demonstrating that abatement is completed for the serious violations that were issued by the Agency for lack of fall protection and installation of handrails on stairways.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  Employees conducting drywall operations on the fourth (4th) floor were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately thirty (30) feet.  The F&H Drywall LLC company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number #634298, citation number one, item number one and was affirmed as a final order on January 16, 2013, with respect to a workplace located at 14901 W. Library Lane, New Berlin, WI.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  (a) Employees were using stairwells with four or more risers without a handrail in place.  The F&H Drywall LLC company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number #634298, citation number one, item number three and was affirmed as a final order on January 16, 2013, with respect to a workplace located at 14901 W. Library Lane, New Berlin, WI.
