29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about August 31, 2016, and at times prior thereto, employees were working in an excavation approximately 12 feet deep and 44 feet long without a ladder or other safe means of egress, exposing employees to the hazards of cave-ins.

29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that was consistent with the recommendations of the manufacturer: On or about August 31, 2016, and at times prior thereto, employees were laying a sewer line for the Maple Park Utilities project using aluminum hydraulic shoring for employee protection. The excavation was approximately 12 feet deep and the shoring was only used for the upper 6 feet of the excavation leaving the lower 6 feet unprotected violating the manufacturer's tabulated data, exposing employees to the hazards of cave-ins.
