On 8/29/2016, workers from Crane Exteriors were installing shingles on a roof of a commercial building located at 5987 Mid Rivers Mall Road in Cottleville, MO. The foreman was not wearing eye protection while using a pneumatic nail gun.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On 8/29/2016, workers from Crane Exteriors were installing shingles on a roof of a commercial building located at 5987 Mid Rivers Mall Road in Cottleville, MO. The foreman and five employees were installing roofing materials on the roof, which was approximately nine feet above the ground, without using guardrail systems with toeboards, safety net systems, or personal fall arrest systems.
