29 CFR 1910.212(a)(2): Guards were not affixed to the machine in a manner such that the guard itself does not present a potential hazard. On or about August 24, 2016 - An employee engaged in bagging activities in the packaging area by the bagger/stuffer machine #4, was exposed to amputation hazards in that lockout tagout procedures were not utilized prior to attempting to unjam a white Teflon plastic paddle.
