29 CFR 1926.25(a):  During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  a) At the New Milestone Construction, residential construction project, construction debris and scrap plywood/lumber was not cleared from work areas, poor housekeeping.

29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity.  a) At the New Milestone Construction residential construction project, the yellow extension cord used for temporary wiring had reverse polarity, the hot conductor was connected to the neutral conductor.
