29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    On or about August 29, 2016, employees in the Flow-Thru Department performing servicing activities were exposed to amputation hazards when cleaning salt from the Dual G Zilla Line Mobile Salter Machine.  The employer did not adequately train all employees that were authorized by the nature of their assignments to lockout machines when cleaning, repairing, and/or setting up machinery to eliminate or reduce an amputation hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    On or about August 29, 2016, employees in the Flow-Thru Department performing servicing activities were exposed to amputation hazards when cleaning salt from the Dual G Zilla Line Mobile Salter Machine.  Employees performing these servicing activities were authorized employees and the employer did not ensure each employee affixed a personal lock on a group lockout device before cleaning, repairing, and/or setting up machinery to eliminate or reduce an amputation hazard.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about August 29, 2016, an employee was performing servicing activities on a Dual G Zilla Mobile Salter Machine while cleaning salt from the machine and the employer did not ensure the following actions were taken prior to servicing activities:     (a) Turn off and shut down the machines,  (b) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (c) Affix lockout or tagout devices to each energy isolating device,  (d) Verify that isolation and deenergization of the machine or equipment have been accomplished.    Employees were exposed to caught-in hazards likely to result in the amputation of hand(s) and fingers.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
