29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about August 30, 2016, and in times prior thereto, the area under a PACMAC 9500 was open allowing operators to reach under the machine exposing their hands to the hazards of coming in contact with the 100 psi pneumatic clamping jaws , cutting knife, 240 volts electrical, rollers and heating elements of the machine. The PACMAC is located in the bagging area and is used for packaging chicken parts.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of electric equipment, such as parts that were broken, bent, cut or deteriorated by corrosion, chemical action, or overheating: On or about August 30, 2016, and in times prior thereto, the base of the enclosure for a 208 Volt transformer was corroded leaving an opening at the base exposing workers to an electrical hazard. The transformer was located in the Main Switchgear Room.

29 CFR 1910.303(e)(2): The marking was not of sufficient durability to withstand the environment involved: On or about August 30, 2016, and in times prior thereto, the marking on a 45 KVA, 208 Volt transformer used the energize a whole chicken bagger were damaged exposing workers to an electrical hazard. The transformer was located in the main switchgear room creating a hazard of electrical burns to workers.
