Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 8/30/2016, employees from Cordoncillos Installations LLC were installing shingles on a roof of a two story residence located at 2150 Roselake Circle, Cottleville, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The foreman and two employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On 8/30/2016, employees from Cordoncillos Installations LLC were installing shingles on a roof of a two story residence located at 2150 Roselake Circle, Cottleville, MO. The foreman and two employees were using three ladders to access the roof. The ladders were not extended three feet beyond the landing or edge of the roof.
