29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Fabrication Shop - On or about August 10, 2016, the point of operation was not guarded on the RAS Press Brake type #61.31, machine #10/5, foot pedal activated, which resulted in a compression injury to an employee's left forearm. b) Fabrication Shop - On or about August 26, 2016, the point of operation was not guarded on the RAS Press Brake type #61.31, machine #10/5, foot pedal activated. which resulted in an amputation injury to an employee's middle fingers of both hands.
