29 CFR 1910.176(b): Storage of material created a hazard: a) On or about September 12, 2016 in the Baling Department; the paper roll stubs were not stored in a manner which would prevent them from tipping over, exposing employees to struck-by hazards. U.S. Paper Converters, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.176(b), which was contained in OSHA inspection number 1081262, citation number 2, item number 1 and was affirmed as a final order on September 19, 2015, with respect to a workplace located at 4808 Converters Ave. Appleton, WI 54915.

29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose: a) On or about September 2, 2016 the flexible cord powering the scale near the baler had been repaired with black electrical tape and not an equivalent insulator as the cord sheathing.
