Such protection did not consist of scaling to remove loose material; installation of protective barricades at intervals as necessary on the face to stop and contain falling material; or other means that provide equivalent protection: a) Subbera Road, Lockport, NY - On or about 9/1/16, loose rock and soil on the excavation face was not contained exposing employees to being struck by the loose material. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): a) Suberra Rd, Lockport NY - On or about 9/1/16, two employees were in an excavation loading the carrier pipe into the casing. The excavation was approximately 15' 4" deep with near vertical walls and not protected from cave-ins. NO ABATEMENT CERTIFICATION REQUIRED
