29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about August 13, 2016, employees cleaning the sensors on the palletizer were exposed to caught-in and amputation hazards from the piston and diverter arm, because the employer considered emergency stops as safe guarding equipment. Employees who performed servicing and maintenance on the palletizers were not required to turned off or shut down, using the procedures established for the machine or equipment.

29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): On or about August 13, 2016, an employee was exposed to caught-in and amputation hazards when testing or positioning the palletizer because the employee did not check all employees have been safely positioned or removed from the infeed conveyor belt.
