General-duty citation text
29 CFR 1926.1430(g)(2): The employer did not provide refresher training in relevant topics for each employee when, based on the conduct of the employee or an evaluation of the employee's knowledge, there is an indication that retraining is necessary. a) The Hanover Project, 300 Village Drive, King of Prussia, PA - On or about August 31, 2016, employees were working within a wood framed residential structure when the overhead trusswork failed, due to the placement of plywood sheets and boxes of nails, which did not have the necessary adequate temporary Top Chord Temporary Lateral Restraint (TCTLR) or bracing installed prior to loading. Follow recommendations of the Truss Plate Institute and Wood Truss Council of America BCSI-B2 or the BCSI-B2 Summary sheet Guide for Installation and Temporary Bracing, that was provided with the trusses by the manufacturer. These recommendations would include but not be limited to the following: 1. Install TCTLR as specified in steps 1 through 5 with maximum 8 feet on center as specified in table B2-1. 2. Install sheathing immediately as stated in the Step 5 regarding installing top chord diagonal bracing. 3. Install temporary bracing throughout, as specified in step 5 thru 8. 4. Pre-assemble trusses for installation as shown in alternate methods on p5, or as documented in BCSI-B11.
