29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) 800 Hanover Road - The employer, who allows employees to voluntary wear disposable respirators in the work place did not review information contained in Appendix D with employees, thereby exposing employees to respiratory hazards on or before October 4, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.243(b)(2): Airhose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected: a) Paint Spray Booths, Sanding station - Employees at the facility are operating air pressure hoses for spraying, sanding or other woodworking projects which have torn outer jackets or in some areas missing the outer jacket completely thereby, allowing employees to operate those airlines which could rupture causing workplace injuries.
