29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about 08/22/2016 at the "Ovation Line"; where packaging equipment was not shut down, deenergized and locked out during an "unjamming" operation when an employee attempted to clear jammed wrapping material. Employees were exposed to a shear point on the machine which was cycling continuously.    ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    a) On or about 09/09/2016 at the Ovation line; where maintenance employees were exposed to arc flash injuries while troubleshooting "live" circuits, which were energized at 575 volts AC. Employees were not wearing protective eye/face protection, protective gloves, or arc flash rated clothing and did not have shirts buttoned all the way to the top with the sleeves rolled all the way down.
