29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a)  At the jobsite, on the street side of the building (West), on the South side of the building, and on the East side of the building, on or about 8/31/16:  The employer did not ensure that floor to celling window openings on the second floor, 8 feet from grade and window openings with sill heights 12 inches from the deck and 8 feet from grade, were fitted with guardrail systems.  a)  At the jobsite, on the street side of the building (West), on the South side of the building, and on the East side of the building, on or about 8/31/16:  The employer did not ensure that floor to celling window openings on the third floor, 16 feet from grade and window openings with sill heights 12 inches from the deck and 16 feet from grade, were fitted with guardrail systems.
