29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle:   a) Production Floor: An employee sustained an amputation hazard while threading plastic through the rollers on the Pre-Stretch # 6 machine without utilizing lockout/tagout procedures, on or about on 8/27/2016.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Floor:  The employer did not ensure that an employee was protected from an amputation hazard through machine guarding of rollers and other rotating parts. On 8/27/2016. Israel Hernandez sustained an amputation while threading plastic through the rollers on the Pre-Stretch # 6 machine.
