29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) NSWC Norfolk Naval Station, 1052 Pocahontas St., Bldg V-47, Norfolk, VA 23511: On August 23, 2016, at approximately 8:30 a.m., a mechanic was caught between a bale and the ship's deck, which resulted in a partial finger amputation. The employer did not report the amputation to the Occupational Safety & Health Administration until August 25, 2016. No abatement documentation or certification required.
