29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:    On or about September 1, 2016, at the trenching site located on the corner of Cameron and 49th West Ave. Tulsa, OK, a competent person was on site but did not conduct an inspection of a trench to ensure conditions did not exist that could result in possible cave-ins, exposing employees to the hazard of being crushed by falling soil.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):      On or about September 1, 2016, at the trenching site located on the corner of Cameron and 49th West Ave. Tulsa, OK, no protective devices were in place to protect employees working in a trench that was more than 5 feet in depth, exposing employees to the hazard of being crushed by fall soil.
