29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a. Facility: 355 Rt. 125 Epping, NH - On or about September 7, 2016, employees did not have a medical evaluation prior to initial use of respirators for activities involving spray foam insulation application and application clean-up following spraying. The employees wore full face supplied air respirators, and were exposed to respiratory hazards including, but not limited to, methylene bisphenyl isocyanate (MDI). Instance deleted with ISA b. Facility: 355 Rt. 125 Epping, NH - On or about September 7, 2016, employees did not have a medical evaluation prior to initial use of respirators for activities involving fiberglass and cellulose work. The employees were required to wear filtering face piece N95 dust mask respirators. c. Facility: 355 Rt. 125 Epping, NH - On or about September 7, 2016 workers voluntarily wearing 3M 6001 full face piece type respirators were not provided a medical evaluation prior to use.

Facility: 355 Rt. 125 Epping, NH - On or about September 7, 2016, employees were not provided with a respirator fit test prior to initial use of 3M full face-piece respirators use during scraping/cleaning work following spraying application. Instance deleted with ISA b. Facility: 355 Rt. 125 Epping, NH - On or about September 7, 2016, employees were not provided with a respirator fit test prior to initial use to ensure that the respirator provided effective protection. The employees were required to wear filtering face piece N95 dust mask type respirators during work activities involving fiberglass and cellulose.
